Institutional insurance coverage - looking for the Current (08 Nov 2010) regs

What are (a) definitions of small business that may limit coverage (b) reasons for denial of coverage if individual has a continuation letter (c) reasons for denial of coverage if individual does NOT have a continuation letter (d) reasons for denial of employment (if the HR department is so stupid as to put person's cancer status as reason for not employing) Thanks. v
